急性上消化道出血是指屈氏韧带以上的消化道疾病引起的出血,为临床常见急症之一,主要表现为黑便、呕血、头昏、心悸等,大量出血还可导致失血性休克而危及生命。出血不仅导致病人的生理变化,还对病人的心理产生巨大的影响。上消化道出血由于发病急,症状变化快,病情较危重,患者突然面对疾病威胁时,会产生生理、心理、情绪等方面的变化,这些负性反应又会加重疾病的进程及病情,影响疾病的治疗[1]。我科对2011年3月~2012年12月收治的138例上消化道出血患者随机分成对照组66例和干预组72例,通过对72例患者综合性心理干预后观察患者前后出血量明显减少,总有效率明显增高。现报道如下:

1 资料和方法

1.1 一般资料 选择年龄在18~78岁,临床症状为“呕血和(或)黑便”的上消化道出血患者,由胃镜检查为活动性胃溃疡或十二指肠溃疡出血,以及糜烂出血性胃炎"同时排除恶性肿瘤引起的出血、严重心肺肝肾功能不全者、妊娠和哺乳妇女、食管胃底静脉曲张破裂出血或药物过敏者。将138例上消化道出血患者随机分成2组,其中,对照组66例中,男35例,女31例,平均年龄(58.1±4.3)岁,胃溃疡28 例,十二指肠溃疡22 例,糜烂出血性胃炎16 例。干预组72例中,男 38 例,女 34 例;平均年龄(56.3±5.2)岁;胃溃疡32例,十二指肠溃疡 25例,糜烂出血性胃炎 15 例。两组性别、年龄、病种等资料经统计学处理均无显著性差异(p>0.05),具有可比性。

1.2 方法

1.2.1 制订护理干预方案 病人入院后均给予及时有效的治疗。对干预组患者根据病人临床表现、诊断结果、既往病史、性格特征、发病诱因、生活习惯、恢复状况,家庭状况等做出综合评估,结合病人具体情况,对病人心理特点分析,得出以下几点心理状态并有针对性的给予综合性心理护理:

恐惧型表现为惊慌失措,恐惧不安,多疑烦躁,情绪不稳,反复询问病情。此类患者特别是初次出血者,看到自己大量呕血或便血,直接感官刺激极易产生恐惧心理,对疾病缺乏认识,担心病情较重,预后不良,救治无望 [2]。护理过程中,要对患者进行耐心的病情解释和心理疏导,要保持镇静不惊慌,以和平的心态、良好的情绪去影响患者,及时清理呕吐物和排泄物,保持室内干净、整洁、有序,操作处置要稳、准、快[3],忙而不乱,让患者对护士有信赖感,使其放松紧张情绪,感到安全舒适。

抑郁型表现为情绪低落,沉默寡言,对外界事物不感兴趣,沮丧、闷闷不乐或独自落泪、厌世,甚至绝望、拒绝治疗等。此类病人多为多次出血、疗效不佳、经济不济,体质虚弱,加上某些操作带来的痛苦,使病人长期处于痛苦绝望之中。对这类病人护理人员应及时掌握心理变化,多与家属沟通,详细解释每天的治疗护理措施和计费清单,告知新农合或医保优惠办法、报销比例以及也可先治疗后交费,减轻心理上的负担,同时告知不良情绪是疾病发生和加重的诱因,使患者保持舒畅愉悦情绪。在实施各项操作时要做到慌而不乱、急而不匆,适时实事求是地使用积极鼓励性语言,并适当地介绍治疗成功的案例,请类似案例现身说法。

焦虑型表现为烦躁不安,心神不宁,敏感多疑、睡眠不佳,心跳加快,血压升高、食欲不振。常因病情来得急,对相关治疗不予理解,毫无思想准备,对疾病的担心与挽救生命的矛盾而产生了严重的心理冲突。护理人员要把患者急于了解的病情、检查结果、治疗方案、护理措施及预后、饮食等方面的内容告诉患者,使其对所患疾病有所认识和了解,通过症状改善事实让患者看到疾病治愈的希望,保持乐观情绪,利于配合治疗。并与患者多接触多交谈,多沟通,做好基础护理,生活护理,做到体贴入微,同时让家属陪护在身边,精心照顾,做好安慰,使患者放松心情,安心治疗。

轻视型表现为症状不明显、出血程度不重、自感平素体健,病情不重,有盲目乐观的侥幸心理,不遵医嘱,不配合治疗,不按要求进食或过早活动或急于出院。对这类病人护士要告知患者疾病发生发展规律及饮食休息对疾病转归的重要性,嘱其禁烟酒及咖啡、浓茶,忌食刺激性、粗硬食物,忌暴饮暴食,帮患者建立科学饮食和健康的生活方式[2]。

1.2.2 临床效果评价: ①痊愈: 临床症状消失,出血停止,休克纠正,大便潜血阴性;②有效: 临床状和出血有不同程度改善,休克纠正,大便潜血阳性; ③无效: 临床症状和出血无变化或加重。痊愈率加有效率为总有效率。

3 讨论

上消化道出血因发病急,病情重,病因不明,视觉上的刺激,病情反复造成沉重的经济负担等,患者多表现焦虑、恐惧、抑郁,灰心失望,甚至绝望,那么加强心理护理,积极对患者的消极心理进行干预,使患者树立战胜疾病的信心就显得尤为重要。而一旦发生上消化道出血病状,心理和情绪就会出现障碍及变化,直接影响疾病的发展与转归。故护理人员应及早、主动地进行心理分析,及时采取相应的护理对策。而患者也希望在护患沟通中获得与健康相关的知识以配合治疗,从护患沟通中得到心理上的安慰[4]。在药物治疗同时给予心理疏导、护患沟通、健康宣教,使患者处于良好的心理状态,心理护理工作应以权威性的解说答疑解惑,巧妙积极的语言和暗示疏导、宽容和抚慰病人,引导重建人生观,对不同的病人采取相应的心理护理措施。这样在抢救治疗及护理康复过程中,患者就能表现出良好的心理素质,并能主动配合治疗与护理的全过程,使病情恢复加快,提高治愈率。

无创机械通气是急性左心衰患者的有效辅助治疗方法之一,可明显改善患者的呼吸状况,保证有效通气,可明显缓解患者的急性左心衰状况,改善心肌功能,进而明显提高患者的生活质量,改善患者的预后[1-2]。为了探讨护理干预在急性左心衰患者使用无创机械通气治疗中的影响,本院选取2011年11月~2013年11月诊治的急性左心衰患者142例,均行无创机械通气治疗,根据护理方案分为两组,行常规护理和护理干预,针对两组患者的主诉恐惧情况、主诉配合情况、主诉并发症情况进行对比分析,现报告如下。

1.资料与方法

1.1资料

本院2011年11月~2013年11月诊治的急性左心衰患者142例,年龄为47~81岁,平均年龄为67.3±5.9岁,其中男性108例,女性34例。患者均符合急性左心衰的诊断标准,且经临床表现和实验室检查确诊,排除患有其他器质性病变、血液病、免疫性疾病的患者,均行无创机械通气治疗。根据护理方案分为两组,常规护理患者71例为对照组,护理干预患者71例为观察组,两组患者的基础资料(性别、年龄等)差异均无统计学意义(p>0.05),具有可比性。

1.2方法

观察组患者实施护理干预:⑴基础干预。护理人员采用和蔼可亲的语言和患者进行交流,建立良好的医患关系,取得患者的信任,通过沟通与交流,帮助患者消除焦虑、抑郁、恐惧、害怕、紧张、担心等不良心理因素,帮助患者建立配合治疗的信心和决心,建立良好的干预基础。⑵情绪干预。向患者讲解疾病的相关知识和治疗的有效方法,使患者了解到疾病治疗的目的和意义,使其可以正视疾病的治疗,在治疗过程中,患者出现恐惧感时要给予及时的安慰和鼓励,采用支持性语言和肢体语言稳定患者的情绪,提高患者的耐受性,消除患者的不安状况,减轻患者的恐惧病症。⑶行为干预。指导患者在治疗过程中采用端坐位,将头部与肩颈处于同一水平位置,将头后仰,确保呼吸道通畅,佩戴合适的面部口鼻罩,调节松紧度,设定吸气相压力由6cmH2O开始,设定呼气相压力由2cmH2O开始,增到患者的耐受,要确保患者的面部皮肤无受损,并在皮肤接触部位放置纱布,减轻患者的压迫感,在皮肤接触部位涂抹凡士林,3h更换1次,每次20min,给予患者局部皮肤按摩等。⑷病情监护。注意观察患者的呼吸节律和心率状况,确保主动呼吸和呼吸机同步,注意保持患者的呼吸道通畅,在加湿器内添加蒸馏水,将气体湿化后进入气道,指导患者掌握正确的呼吸方法。对照组患者实施常规护理。患者在治疗过程中,给予患者常规的基础护理,保证患者的病房通风良好和环境的整洁卫生,及时更换患者的床单被褥,只要患者实施正确的饮食和作息等。

1.3评定标准

主诉恐惧情况的评定标准:⑴轻度。患者不回避治疗,无明显的恐惧表现。⑵中度。患者有恐惧表现,曾试图回避治疗。⑶重度。患者有强烈的恐惧表现,尽力回避治疗,在护士帮助下才能完成治疗。

主诉配合情况的评定标准:⑴积极。患者可以积极主动配合治疗。⑵一般。患者可以基本配合治疗。⑶较差。患者需要在护士帮助下勉强完成治疗。

主诉并发症情况的评定标准:⑴轻度。患者面部受压处皮肤发红和疼痛,停机5~10min后可恢复正常。⑵中度。患者主诉有腹胀感,可耐受,面部受压处皮肤发红和疼痛,停机5~10min不能恢复正常。⑶重度。患者主诉腹胀明显,行胃肠减压和肛管排气后恢复,面部受压处皮肤破损。

[Abstract] Objective To curative effect analyze cimetidine and interferon combination of trace element zn in pediatric infectious diarrhea rotavirus. Methods 67 cases of children with infectious diarrhea rotavirus in our hospital from November 2015 to December 2016 were selected and divided into control group (implementation of cimetidine and interferon therapy) and treatment group (the implementation of cimetidine associated with interferon therapy trace element zine). 临床上,轮状病毒感染性腹泻在儿科中具有较高的发病率,引发本病的病原体主要为轮状病毒,此病的主要临床症状表现除了腹泻之外同时普遍并发有脱水、发热等症状,严重的甚至出现代谢紊乱以及酸中毒,通常好发于秋冬季节,若无法及时诊断并接受针对性治疗,严重的甚至可直接引发死亡[1]。现阶段,临床上对本病的发病机制尚无法明确,因而无统一的且具有明显效果的治疗标准[2]。为此,我院引入西咪替丁与干扰素联合微量元素锌治疗,现将疗效报道如下。

1 资料与方法

1.1 一般资料

67例均为我院自2015年11月~2016年12月期间收治的轮状病毒感染性腹泻患儿,纳入标准:临床症状均符合本病的症状表现,同时经病原学检查确诊。经医院伦理委员会审批后,将67例患儿随机分为治疗组与对照组两组,对照组32例,其中男17例,女15例;年龄0.7~4.5岁,平均年龄(1.6±0.8)岁。治疗组35例,其中男19例,女16例;年龄0.8~4.4岁,平均年龄(1.6±0.7)岁。两组患儿在基线资料方面对比差异无统计学意义(P>0.05),具有可比性。

1.2 方法

两组均给予常规检查,同时结合脱水的严重程度给予补液治疗,给予发热以及呕吐者对症治疗,注意两组均不能给予抗生素治疗。基于此,对照组采用西咪替丁注射液(湖南汉森制药股份有限公司;H43021799)10~15mg/(kg・d)治疗,给药方式为静脉滴注,同时增加重组人干扰素α1b [安徽安科生物工程(集团)股份有限公司;S20040010]治疗,给药方式为注射,剂量为100万U/次,1次/d。在此基础上,治疗组联合葡萄糖酸锌口服液(湖北福人金身药业有限公司;H20065022)治疗,给药方式为口服,每支的含锌量为0.03g,1支/d,两组的疗程均为10d。

1.3 观察指标及评定标准

记录两组大便及体温恢复时间、脱水纠正时间。记录两组面色潮红、烦躁以及腹泻加重等不良反应情况。对两组治疗效果进行评估,若3d后,症状消失,同时粪便次数以及形状均正常则评定为显效;若3d后症状明显缓解,同时粪便次数及形状明显好转则评定为有效 ;若3d后症状未见任何缓解,同时粪便次数及形状未见任何改善则评定为无效。总有效率=100%-无效率。

1.4 统计学分析

使用SPSS19.0统计学软件处理数据,两组计数资料对比行χ2检验,两组计量资料对比行t检验,用P

2.1 两组治疗基本情况比较

治疗组治疗总有效率为97.14%,明显高于对照组的71.88%(P

2.3 两组不良反应比较

治疗组不良反应发生率为2.86%,与对照组的不良反应发生率6.26%对比差异无统计学意义(P>0.05),详见表3。

3 讨论

临床上,感染性腹泻属于儿科中常见的一种肠道感染性疾病,出现腹部疼痛以及粪便不成形等为本病的主要症状表现[3-4]。人体小肠内部的载膜绒毛上皮细胞中存在一种可以与轮状病毒外壳中含有的VP4相互结合的病毒原体,因此小肠成为了轮状病毒侵入的重要部位[5]。待病毒继续侵入机体上皮细胞之后,便会诱发机体内的大量非结构蛋白糖基化,进而引发细胞膜发生紊乱现象,例如钙离子通透性,一旦机体细胞内的钙离子出现大量增多的现象,便会引发高钙低氯症状,进一步可增加病毒非结构蛋白NSP3抑制蛋白正常合成的风险,进而将影响小肠上皮细胞的运行,严重的甚至可导致其脱落,极大的减少了双糖转化为单糖过程中所需的肠载膜上绒毛酶的数量,随着时间的延长,双糖大量堆积在肠道内,便极易增加肠腔内渗透压,进而诱发腹泻、呕吐及脱水等症状,严重的可并发水电解质紊乱以及酸中毒等严重威胁患儿生命的症状[6-7]。

西咪替丁是一种H2受体拮抗剂,能够显著抑制因组胺或者胃泌素引发的胃酸分泌,进而避免受损肠黏膜长期处在酸性环境中而对其起到保护作用,并有利于为绒毛的修复创造良好条件[8-9]。此外,此种药物还具有免疫调节作用,能够通过提高患儿机体内的T淋巴细胞的转化率而起到增强其免疫力的效果,进而能够避免炎症反应诱发的一系列继发性损伤问题[10-11]。干扰素则具有免疫调节以及抗病毒效应等多种生理功能,在本病的治疗过程中具有显著的价值[12]。刷状缘中大部分酶类均对锌元素存在较强的依赖性,同时锌离子能够显著的抑制小肠黏膜的增生,且能够促进其完全角化,进而能够对肠黏膜起到保护作用,此外,还可抑制胃肠激素分泌,增加肠内钠氯离子的吸收能力,进而能够对肠腔的渗透压起到降低的效果[13]。除此之外,锌离子还具有免疫调节以及抑菌等多种功效。特别值得关注的是,我院在采用西咪替丁与干扰素常规治疗的基础上增加微量元素锌的一个理论基础是此类型的患儿血清中锌代谢有异常的现象[14-16]。

综上,将西咪替丁与干扰素联合微量元素锌应用于轮状病毒感染性腹泻患儿的治疗中,可以快速消除患儿的症状,促进其康复,具有疗效理想,安全性高的特点,值得临床上推广及应用。
